Recipients or subrecipients must examine a program participant's income INITIALLY AND AT LEAST ANNUALLY THEREAFTER to determine the amount of the contribution toward rent payable by the participant, and adjustments to that contribution must be made as changes in income are identified. For occupancy charges the examination is initial, and where family composition changes or the resident's income decreases during the year the resident may request an interim reexamination and the charge is adjusted accordingly. As a condition of participation each program participant must agree to supply the information or documentation necessary to verify income, and must provide information at any time regarding changes in income or other circumstances that may change the contribution. For each participant who receives housing assistance where rent or an occupancy charge is paid, the file must document annual income with: the income evaluation form specified by HUD and completed by the recipient or subrecipient; source documents such as the most recent wage statement, unemployment compensation statement, public benefits statement or bank statement for assets held and income received before the evaluation date; to the extent source documents are unobtainable, a written statement by the relevant third party or the written certification by intake staff of that third party's oral verification; and, only to the extent both source documents and third-party verification are unobtainable, the participant's own written certification of the income reasonably expected over the 3-month period following the evaluation.
