The PHA must obtain and document in the family file third-party verification of reported family annual income, the value of assets, expenses related to deductions from annual income, and other factors that affect the determination of adjusted income or income-based rent, or must document in the file why third-party verification was not available. For a family with net family assets at or below the self-certification threshold the PHA may accept, for purposes of recertification of income, the family's declaration under 24 CFR 5.618(b), except that the PHA must obtain third-party verification of all family assets every three years. The family must supply any information the PHA or HUD determines is necessary to administer the program, including any requested certification, release or documentation, and any information requested for use in a regularly scheduled or interim reexamination; all information supplied by the family must be true and complete. As a condition of admission and of continued assistance the PHA must require the family head, and such other family members as it designates, to execute a consent form authorising any depository or private source of income, or any federal, state or local agency, to furnish or release the information the PHA or HUD determines to be necessary, and the use or disclosure of information so obtained is limited to purposes directly connected with administration of the program.
