Adult members of an assisted family must authorise the owner to request independent verification. Each family member at least 18 years of age, and the head of household, spouse and co-head regardless of age, must sign and date form HUD-9887 (Notice and Consent to the Release of Information to HUD and to a PHA) and form HUD-9887-A (consent to verification by owners), at the initial certification, at move-in and at each annual recertification, and again when a new adult member joins the household. Each adult must sign whether or not he or she has income. The owner must keep the original signed forms in the tenant file and give the family a copy of each form signed. Both HUD forms and the owner's own verification forms expire 15 months after they are signed, and the owner must confirm they have not expired before processing a verification. A current form HUD-9887 must be on file before the owner accesses EIV employment and income information for a tenant, although it is not required to use the EIV verification reports, including the Existing Tenant Search for applicants.

Change 4's own transmittal records the amendment: 'Paragraph 3-11.A.1 - Added co-head must sign and all must date the form at initial certification and each recertification.' The co-head signature and the dating requirement therefore date from 2013-08-07, not earlier.
