The tenant file must hold the signed consent forms, the EIV Income Report, the HUD-50059 and third-party verifications, kept for the tenancy plus three years and disposed of securely

The owner must keep in the tenant's file at the project site: all original signed forms HUD-9887 and HUD-9887-A; a copy of signed individual consent forms; a copy of the EIV Income Report regardless of whether any income is reported for the household, together with the HUD-50059 and any other documentation supporting income and rent determinations; and third-party verifications received from third-party sources. The owner must maintain documentation of all verification efforts throughout the term of each tenancy and for at least three years after the tenant moves out. The file should be available for review by the tenant on request, or by a third party with the tenant's signed authorisation, but EIV data of one adult household member may not be shared with another adult household member without that individual's written consent. Files and records must be disposed of in a manner that prevents unauthorised access to personal information.

5-23.A.3 -- the EIV Income Report in every file whether or not income is reported -- was added by Change 4 and is one of the most common Management and Occupancy Review findings.
